Autologous fat transplantation is a process that has been used for several years in plastic surgery to improve the quality of tissues (hard tissue after radiotherapy or to reshape a breast after cancer surgery). This process also improves the appearance of scars and promotes hair regrowth on the scalp.

Recent publications from the last ISHRS congress which took place in October 2020, report hair regrowth after this kind of graft which sometimes improves disappointing hair transplant results or allows a scar to be grafted.

Usually, a scar is an evolutionary process that lasts between six months and a year. However, scar hypertrophy can appear on some skin which can evolve for at least two years.

In all cases, if the scar is unsightly, treatment can be implemented: surgical alteration of the scar, with or without appropriate medical treatment, or isolated medical treatment.

Indeed, a scarring depression (atrophic post-inflammatory scar) can be filled in by the insertion of fat cells. Certain inflammatory dermatological conditions, when they stop progressing, leave an atrophic scar which is thin and hairless and therefore difficult to correct if they are extensive.

What is a fat graft?

A fat graft (nanofat grafting in scientific literature) or autografting of fatty tissue is a technique consisting in removing small amounts of fat from the body to be reinjected into the target tissue after being repeatedly filtered. This process is already used in plastic surgery to fill the hollow formed by dark circles under the eyes, improve the appearance of scars, fill tissue depressions or reshape a face or breasts.

Indeed, in plastic surgery, autologous fat has been used for several years with success and the common denominator is the improvement of the quality of the tissues in which the fat is injected. The difference is that the quantities injected in the case of a nanograft are very low, the goal is not to reshape an anatomical region, but to improve tissue quality.

In our NHT Europe clinic, we use this advanced procedure, derived from this fat autograft technique: We successively filter the fat sample (nano fat graft) more and more finely, which allows us to obtain higher concentrations of stem cells (mesenchymal stem cells); the analyses of these samples show a total destruction of the adipocytes, but they contain the stromal vascular fraction (SVF) with a concentration of stem cells identical to those obtained by enzymatic digestion of the fat. These will improve tissue trophicity, soften scars and allow hair transplantation to be carried out under better conditions.

What are the indications for a nano fat transplant?

Used initially to improve scars, this procedure also improves the outcome of a hair transplant.

Scars can occur for very different reasons. Sometimes they remain large and visible after an accident or surgery. The same goes for burns, or after certain inflammatory conditions (lichen plano pilaris) where the scars can spread all over the head.

An implantation of your own hair covers the scars and allows you to regain your well-being.

Recent scientific communications (ISHRS Congress October 2020, International Society of Hair Restoration Surgery) also report significant improvement in hair after such a transplant.

We are undoubtedly at the beginning of a new therapeutic concept for baldness and these procedures will certainly become more widespread in the near future.

How does a nanofat graft take place in anticipation of a hair transplant?

In order to improve the fibrous tissue and soften your scar, it is first necessary to perform a nano fat graft.

This procedure is very often used in reconstructive surgery. This innovative technique will allow stem cells to be injected into the area to be treated and thus recreate healthy tissue.

The procedure consists of taking a few cubic centimetres of fat under local anaesthesia, centrifuging it, and filtering it with increasingly fine filters to get a solution containing a high concentration of so-called multipotent stem cells, which are able to recreate many tissues.

A new vascularization is also induced making it possible to consider a possible transplant with a greater chance of success.

The indications for these injections are mainly linear scars and post-inflammatory cicatricial alopecia.

Following this procedure, an FUE micro-graft can be considered. The area is "prepared".

Indeed, without this preparation and on scar tissue, the chances of the transplant taking are random.

In summary: to survive, the graft must be vascularized, therefore implanted in healthy or at least vascularized tissue; this can be done by implanting the graft deeper to avoid the fibrous area or by new growth of healthy tissue by fat injection.

What results can you expect after a nano fat transplant?

The results of these nano fat cell transplants are often surprising.

Very positive for the treatment of scalp scars, progress is being continuously made and will certainly advance the therapeutic possibilities for treating alopecia.

OTHER REGENERATIVE THERAPY: RIGENERA

This is a procedure that allows the harvesting and activation of autologous hair follicle stem cells (HFSC) thanks to a few pre-collected grafts; a mechanical disaggregation is then carried out by rotary filtration of the grafts and the filtrate reinjected by mesotherapy: hair mesograft.

We have obtained a decrease in hair loss with a clear redensification of hair in most cases; only one case required a second procedure two years after the first one for the reappearance of hair loss.

EXOSOMES

The cells of the dermal papilla secrete growth factors that facilitate the differentiation of stem cells.

Recent studies show that exosomes derived from these dermal papilla cells regulate the hair cycle and promote the growth phase.

Further studies are needed to better assess the benefit of this therapy.
